Before You’re Admitted
— What to Bring
Being prepared makes everything smoother. Try to bring:
A complete list of your medications — including dosages, frequency, and prescribing doctors. This is the single most helpful thing you can bring.
A list of your known allergies — especially medication allergies, and what reaction you had
Names and contact information for your primary care doctor and any specialists you see
Your insurance cards and a photo ID
A trusted support person — a family member or friend who can be present during physician visits and help you remember what was said
A notepad or your phone to write down questions and answers
